A bloody chainsaw held by an unknown assailant was a feature of Exhumed's first album from 1998, "Gore Metal". It wasn't until the start of this decade — when one of their roadies created the character of Dr. Philthy by wrenching the chainsaw from the cold, dead hands of the band, pulling on bloody hospital scrubs and a soiled lab coat.
